What a syndesmosis injury is
A syndesmosis injury, often called a high ankle sprain, affects the ligaments that bind the two lower leg bones together just above the ankle. It is different from the more common sprain on the outside of the ankle, tends to be felt higher up, and usually takes longer to settle, which is why a clear plan matters.
Why it takes time
The structures involved take longer to recover than a typical ankle sprain, and the demands of your sport shape the timeframe. Getting the diagnosis right early helps set realistic expectations.
How we approach it
The podiatrist examines the ankle and lower leg, checks movement and stability, and looks at how you load and move. The plan typically includes staged loading, strength and balance work, and footwear advice, with imaging or onward referral arranged where the injury calls for it.
Signs people often notice
- Pain above the ankle, towards the lower shin
- Discomfort when the ankle is loaded or rotated
- Pain pushing off or changing direction
- A sprain that is slower to settle than you expected

Questions
Common questions
How is this different from a normal ankle sprain?
It affects different ligaments above the ankle and usually takes longer to settle. The podiatrist can assess it and explain what is involved.
Will I need a scan?
Sometimes. Where imaging or onward referral would help confirm the picture, the podiatrist will arrange it.
When can I return to sport?
This injury rewards patience, so return is staged and guided by your progress. The podiatrist will set out a realistic timeframe.
