Flat Feet & Overpronation
Flat feet or “overpronation” are common findings and do not always need treatment. Problems arise when the way the foot rolls in places extra stress on joints, tendons or other structures higher up the leg.
When flat feet may be a concern
People sometimes seek help when they notice:
- Feet that roll in strongly when standing or walking
- Aches through the arches, ankles, shins or knees
- Shoes wearing out more on one side
- Children with very flat feet, tripping or tired legs
What can contribute to overpronation
Factors may include:
- Natural foot shape and joint flexibility
- Weakness or poor endurance in muscles that support the arch
- Previous injuries around the ankle and foot
- Inappropriate or worn-out footwear
Our role is to decide whether the way the foot moves is actually contributing to symptoms.
How we assess flat feet
Assessment looks at both static posture and dynamic movement. We typically:
- Review symptoms and activity demands
- Observe standing posture and foot alignment
- Assess joint mobility and muscle strength
- Watch walking and, where appropriate, running
- Review current footwear and any previous orthotics
Treatment options
If the way the foot rolls in is contributing to pain or fatigue, management may involve:
- Strengthening exercises for the foot, ankle and hip
- Footwear recommendations to improve support and stability
- Orthotics to assist with load sharing and alignment
- Activity and load advice as part of a musculoskeletal assessment
- For children, monitoring over time through our children’s podiatry service
Flat feet and overpronation patients we see
We see adults worried that flat feet are causing pain, children with very flexible feet and active people concerned about injury risk. Not every flat foot needs intervention, and we are clear when reassurance is all that is required.
